Adur Older Persons’ Network works in close partnership with the Adur & Worthing Community Partnership Team (CPT).

The CPT is a joint team including staff from West Sussex County Council, West Sussex Primary Care Trust, Department for Work and Pensions, and local charity Guild Care.

The CPT will work with an older person to advise or actively put in place a range of measures which will assist that person’s health, quality of life and ability to live independently.
